1. Pre-Enrollment
Our Companion of Care Home Health Inc. administrator collects further patient information and insurance details to prepare a personalized care schedule and secure financial coverage.
2. Admin Consult
Patients are paired with one of our caregivers based on their needs and staff availability. Both parties will be formally introduced and explained care expectations.
3. Assignment
The assigned caregiver will start providing care at the patient's home, according to their personalized care plan until the assignment is amended or canceled.
4. Care
Patients, their families, and our licensed caregivers will submit feedback to measure the quality of care being provided, to allow for necessary adjustments or interventions.
5. Audit
